Privacy Policy and Informed Consent for Eyelash Extensions, Eyelash Lifting, Eyelash Tinting, Threading, and Eyebrow Laminating

I DECLARE:

I understand that as part of the eyelash extension procedure, there is a risk of eye irritation, eye pain, itchy eyes, discomfort, and conjunctivitis following the application of the extensions. In very rare and unlikely cases, there is a risk of infection or blindness.
I agree that if I experience any of these medical conditions as a result of wearing synthetic eyelashes, I will immediately go to my nearest healthcare facility and then contact the independent eyelash extension specialist to have them removed.
I understand that if I have my eyelash extensions removed for the reasons mentioned above, I will not be eligible for a refund of the service fee.
I understand that even if the independent eyelash extension technician applies or removes the eyelash extensions using the proper technique, the instruments, tapes, facial cleansers, gel pads, adhesives, and/or removers used in the procedure may irritate my eyes and may require medical attention and follow-up, as well as subsequent removal of the eyelash extensions.
I understand and agree to follow the care instructions provided by the independent eyelash extension specialist regarding the use and care of my eyelash extensions.
I am aware and acknowledge that failure to follow these instructions may result in the eyelash extensions falling out, becoming damaged, and/or having their lifespan shortened, as well as causing eye and/or skin irritation.
I understand and consent to having my eyes closed and covered for a period of 30 to 120 minutes while eyelash extensions are applied.
I have informed the independent eyelash extension specialist of the following conditions:
- Current use of contact lenses
- Current use of oil-based products around the eyes
- Current use of eye drops
- Current allergies or sensitivities to the following materials: surgical steel, ethyl cyanoacrylate, polyester, adhesive tape, facial cleansers, adhesives and removers, latex.
- History of claustrophobia
- History of recurrent infections of the tear duct
- History of dry eyes
- Recent history of chemotherapy
- Other medical conditions that may interfere with the application and maintenance of eyelash extensions.
I agree to the following instructions for the care of my eyelash extensions:
- Avoid using mascara and eyeliner.
- Avoid dyeing or curling your eyelash extensions.
- Avoid using oil-based cosmetics.
- Avoid constantly pulling or rubbing your eyelash extensions.
- Avoid getting the product in your eyes for 24 hours after application.
I understand that this agreement will remain in effect for this service and all future services provided by the independent eyelash extension professional at salons booked or promoted through Natiya Media LLC and listed in its database of active salons at the time of service.
I understand that there are no guarantees regarding the duration of the eyelash extensions and that Natiya Media LLC is not liable for technical errors that occur during the application due to the client’s negligence, such as: arriving at the appointment with makeup on the natural eyelashes and around the eyes, arriving at the appointment without having washed the eye area, failing to respect the appointment time by arriving late or causing delays for the specialist due to reasons such as the use of mobile devices and other voluntary or involuntary movements in the eye area.
I understand that Natiya Media LLC offers a follow-up check once the eyelash extensions have been applied and before the 5-day period expires, to replace any extensions that may have fallen out, provided there are three or more extensions per eye per day. This review will not apply in cases of failure to follow the instructions for caring for eyelash extensions or due to service delays caused by the client.
I understand the care instructions and agree to follow them to ensure my eyelash extensions remain in good condition.
I understand that there are many factors that can affect the lifespan of eyelash extensions, such as water, exposure to moisture, weather conditions, activities involving exposure to high temperatures, alcohol consumption, and the use of certain cosmetic products.
I have read and understood all the information contained in this agreement and understand that this consent is legally binding. I am over 18 years of age and consent to the agreement and the treatment. I release the Brand, marketing, and appointment management services owned by Natiya Media LLC from any liability associated with this procedure, which is performed with the utmost attention to safety and the proper application of eyelash extensions, using the tools and products that the technician has been professionally trained to use, and Natiya Media LLC shall not be held liable for any procedure not performed correctly or in accordance with the techniques with which the professionals have been trained by the platform itself.
If I checked the “Consent to Recording” box during the appointment, I authorize the use of these recordings in advertising materials and grant Natiya Media LLC the rights to these images.
I hereby confirm that I have read and understood the above statements and agree to them.

Special Terms for Models, Internships, and Discounted Services

I understand and agree that, when receiving eyelash extensions, eyelash lifts, eyelash tinting, threading, or eyebrow laminating services for free or at a discount for modeling or training purposes, I must comply with the following conditions:
- Confidentiality: I agree not to disclose, share, or communicate, directly or indirectly, any confidential information to which I have access during or after the appointment. This includes techniques, processes, products used, and any other aspects related to the services provided. I understand that this confidentiality obligation does not limit my right to share my personal experience, provided that no technical details or information that “Natiya Media LLC” considers confidential are disclosed.
- Social Media Posts: I agree that if I wish to post, share, or comment on social media, blogs, websites, or other public platforms about my experience at the center, I must do so truthfully and respectfully. I understand that I may express my personal opinion, but I agree to refrain from making any statements that may be defamatory, false, or intended to unfairly damage the center’s reputation.
- Content Approval: If I receive the service for free or at a discount for modeling or promotional purposes, and am asked to create specific content (such as photos or testimonials), I agree that such content must be approved in advance by "Natiya Media LLC" prior to publication. I understand that this clause applies only to content generated at the specific request of "Natiya Media LLC" and not to my unsolicited personal opinions.
- Image Rights: If I checked the “Recording Consent” box during the Appointment, I authorize the use of images and videos captured during the service in advertising materials, thereby granting the rights to such images to “Natiya Media LLC.” I understand that I may revoke this consent at any time by notifying “Natiya Media LLC” in writing.

Acceptance of the Cancellation and Refund Policy

1. Canceling and Rescheduling Appointments
- Cancellation and Rescheduling Policy: I understand that I must cancel or reschedule my appointment at least 24 hours in advance. I can make these requests through the online booking system or by contacting us directly by phone or email.
- Reservation Deposit: I agree that
- Guaranteed Reservation: I understand that the 15% deposit guarantees my spot on the salon's schedule and ensures that the stylist will be available for me at the agreed-upon date and time.
- Refunds for Early Cancellations: I understand that if I cancel or reschedule my appointment more than 24 hours in advance, I will receive a full refund of the booking deposit. This refund will be issued via the same payment method used within a maximum of 7 business days.
- Late Cancellations: I acknowledge that if I cancel or reschedule my appointment with less than 24 hours’ notice, the booking deposit will not be refunded. I agree that this policy is intended to compensate for the time set aside for my appointment.
- No-Show: I agree that if I fail to show up for my appointment without prior notice, I will forfeit 100% of the booking deposit. However, if the lash technician has an available slot on the same day and my failure to show up was due to unforeseen circumstances beyond my control, I may use the deposit to reschedule the appointment. I understand that to do so, I must contact the lash artist directly to coordinate the rescheduling, subject to the professional’s approval.
2. Refunds for Services Rendered
- Satisfaction Guaranteed: I agree that if I am not satisfied with the service provided, I must notify you immediately so that any issues can be resolved promptly.
- Refunds After Services Have Been Performed: I understand that services already performed are non-refundable, but the salon is committed to offering me a solution, either by correcting the service or providing a discount voucher for a future visit, depending on the circumstances.
3. Exceptions
- Force Majeure: I acknowledge that in exceptional circumstances, such as medical emergencies or force majeure events, the salon reserves the right to make exceptions to the cancellation and refund policy, at its discretion.
4. Complaints
- Procedure: I understand that if I wish to file a complaint regarding the cancellation policy, refunds, or the service received, I may do so in writing by sending an email to support@lashesoriginal.com. The salon agrees to respond within 7 business days.
5. Governing Law
- Spanish Law: I acknowledge that this policy complies with current Spanish law, including the General Law for the Protection of Consumers and Users and other relevant laws. Any dispute regarding the cancellation and refund policy will be resolved under the jurisdiction of the Spanish courts.

Data Protection

In order to comply with Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and in accordance with the Recommendations and Guidelines issued by the Spanish Data Protection Agency (AEPD), PLEASE BE ADVISED:
The personal data requested and provided by you is incorporated into a privately owned database for which Natiya Media LLC is the sole data controller and recipient. Only the data strictly necessary to properly provide the requested services will be collected; it may be necessary to collect contact information from third parties, such as legal representatives, guardians, or designated caregivers. All collected data is subject to a confidentiality agreement and legally established security measures, and under no circumstances will it be transferred or processed by third parties, whether individuals or legal entities, without the prior consent of the client, guardian, or legal representative, except in cases where it is essential for the proper provision of the service.
Once the relationship between the company and the customer has ended, the data will be archived and retained for a minimum period of 6 years, after which it will remain archived or, failing that, will be returned in full to the customer or their legal representative. The data I provide will be included in the data processing activity titled “Natiya Media LLC Clients” for the purposes of managing the contracted service, issuing invoices, contacting the client, and all other client-related matters, and I hereby give my consent. I have also been informed of my right to exercise my rights of access, rectification, erasure, and objection by submitting a written request to Natiya Media LLC, located at 2201 Menaul Blvd NE stA, Albuquerque, 87107, New Mexico, United States.
To ensure the proper provision of services, independent professionals booked through Natiya Media LLC’s software will have access to the data necessary for confirming appointments and performing the contracted services. Once independent professionals have access to this data, the responsibility for protecting it rests with those professionals. Natiya Media LLC is not liable for any misuse of personal data by independent professionals.
